    Why Your TSH Test Is Not Enough

    The standard thyroid evaluation in Los Angeles County medical practices consists of a single test: TSH, thyroid stimulating hormone.     Thyroid function requires a cascade of events: the pituitary releases TSH, the thyroid produces T4, peripheral tissues convert T4 to the active hormone T3 through deiodinase enzymes, and T3 enters cells to regulate metabolism. Dysfunction can occur at any point in this cascade while TSH remains normal. The most common missed diagnosis is T4-to-T3 conversion failure, where the thyroid produces adequate T4 but inflammation, nutrient deficiency, gut dysfunction, or stress impairs the conversion to active T3.

    Reverse T3 represents another critical blind spot. Under stress, illness, or caloric restriction, the body preferentially converts T4 to Reverse T3, an inactive molecule that blocks T3 receptors. A Los Angeles member can have normal TSH, normal T4, and still be functionally hypothyroid because Reverse T3 is occupying the receptors that T3 needs to activate.

    Thyroid antibodies, TPO and thyroglobulin, identify autoimmune thyroid destruction years to decades before TSH becomes abnormal.
